Home Care

Capital Health Home Care is a Medicare/Medicaid-certified home health agency qualified to provide skilled nursing, physical therapy, occupational therapy, speech therapy and home health aide services. Our goal is to make certain you achieve an optimum level of health and well being under our care.

One of our professionals is always available to address your needs. We have a scheduler and a nurse on call 24 hours a day / 7 days a week / 365 days a year for consultations after hours and on weekends. We encourage you to call us anytime you have a concern but especially when you experience an unexplained change or decline in your health.

After choosing Capital Health Home Care as your home care provider, our intake coordinator will contact you to finalize services. One of our nurse case managers will make an initial assessment visit within 24-48 hours. A team of caring medical professionals will be assigned to you based on your specific needs.

Home health aides can assist with personal care including bathing, dressing, toileting, and light housekeeping. They are not permitted to provide transportation or heavy housekeeping under the Medicare visit program.

Senior Care – Skilled Nursing & Rehabilitation

Our Senior Care division, will begin getting to know new residents individual needs, interests and expectations. Shortly after getting settled into the community, within 72 hours, we schedule a meeting with the resident, family members and key staff to answer any questions, discuss care needs and any specialized programs or requirements.

During these first few days residents will meet a number of staff members who will play essential role in their recovery and help them return to a lifestyle to which they are accustomed. Through this transition period, or at any time in the future, should you or a family member have questions we are always here to assist you.
