Capital Health Home Care launched a Dementia Specialty Program in February of 2016 that is robust and comprehensive.

The program includes:

  • A true understanding of disease progression and challenges
  • Certified Dementia Practitioner training
  • Interdisciplinary team approach including private duty
  • Family education & support
  • Meaningful activities

The program revolves around a Person-Centered Approach to help manage dementia. That means we take the time to get to know our clients in the present and in the past so we can develop a holistic Plan of Care.  We know that treatment involves the whole person – body, mind, spirit and emotions.

Capital Health Home Care opted to forego traditional holiday giveaways in 2015 in favor of providing free personalized iPod Gift Sets to seniors with late stage dementia. Recipients were nominated by members of the healthcare community and selected based on need.  The gift sets included iPod shuffles, head phones, portable chargers and iTunes gift cards.

We plan to make our Melodies & Memories Campaign a standing initiative in our community outreach efforts. We will be back out there in the 2016 holiday season looking for nominees. This was a simple gesture that made a positive impact in the lives of seniors with dementia and their families. It also brought great joy to our team of dedicated and compassionate home care professionals. It truly is more blessed to give than receive.
